Participant Registration Form

Hello and welcome to the Sambão community of friends and family. Thank you for your interest in Carnaval San Francisco 2002. We look forward to a joyous celebration with you as we sing and dance down the streets of the city presenting the beauty and pageantry of our samba school to the people of the world. There are only two requirements for your participation:

• A signed waiver included on this page
• A $10 registration fee which includes membership in Sambão

Beyond that we ask you please consider making a contribution, purchasing the CD of the theme song (the Samba Enredo), and making a deposit toward your costume fee or a souvenir T-shirt.

Sambão is a “grass roots” organization supported by community contributions. We are a California Non-profit Corporation and your contribution is tax-deductable. Any amount will be greatly appreciated and you will send you a letter verifying the tax-deductable contribution.

Please fill out the registration form with your contact information and sign the waiver. If you will be turning in your fee, contribution, or deposit at a later date please so note. In addition to your contact information please write down on the back pagh the names and addresses of friends or family who might be interested in our carnaval activities.

Participation in the 2002 San Francisco Carnaval Parade with Sambão requires the signature of the registering adult or the parent or guardian of any minors.

Waiver:  Permission to participate in the 2002 San Francisco Carnaval Parade, related rehearsals and events is given for myself and/or child as listed below.  I hereby indemnify, hold harmless and release Sambão, its officers, members and volunteers from any and all liability for injury suffered by myself or child arising from or connected with this event.  I assume liability.
